Accident Sequence
The pilot of the helicopter reported that the engine misfired three times during the takeoff roll and subsequently lost partial power. The helicopter descended and settled into a rice field, where the spray booms, already damaged during the landing, became entangled with vegetation. The helicopter then nosed over in the high vegetation, and as the aircraft came to rest inverted, the engine briefly accelerated before shutting down.
Examination Findings
Post-accident examination of the carburetor bowl revealed the presence of rust. Two particles of rust, each measuring approximately 1/4 inch in diameter, were found lying in the bottom of the bowl. No water was detected in the engine or fuel system.
