Event Summary
An aircraft contracted by the State of Washington was conducting fire suppression operations using a water bucket attached to a long line. After refueling at a temporary heliport in Entiat, Washington, the helicopter departed for a work area 15 nautical miles southwest of Entiat. The aircraft did not arrive at its intended destination.
Wreckage and Evidence
The wreckage was found on the eastern slope of a steep mountain range. Investigators discovered evidence of an in-flight fracture of the main rotor mast, which was accompanied by an in-flight breakup of the aircraft. Weather data from the time of the accident indicated strong, gusty winds from the northwest and mountain wave conditions in the vicinity.
Flight Crew Information
Evidence indicated that the copilot was at the controls of the helicopter at the time of the accident. This flight was the copilot's first in the BH-204 model and his first with the operating company.
