An aircraft departing from runway 17 experienced smoke from the right engine and loss of power in both engines, resulting in a landing on rough terrain about a quarter mile southwest of the runway. The aircraft was substantially damaged.

Background

The aircraft had recently changed ownership. During a pre-purchase inspection in Florida, metal particles were found in the oil screens of both engines. The oil was changed and flushed, but after another engine run, metal particles were again present. In May 1989, the right engine was replaced with an engine from another aircraft. The aircraft was then flown to Oakland, California, where it was painted and a new interior was installed. A local mechanic noted metal particles in both engine oil screens and recommended an oil analysis, but the ferrying pilot refused. After a flight to Hawaii, no oil stains were noted on the fuselage before the flight on September 9, 1989.

Event

As the aircraft was departing from runway 17, smoke was observed coming from the right engine. The pilot confirmed a loss of power and made a right turn back toward the runway, then reported that he had lost both engines. The aircraft was extensively damaged during a landing on rough, rocky terrain approximately one quarter mile southwest of the runway threshold.

Investigation Findings

Examination of the wreckage revealed that both engines failed due to detonation. Heavy oil streaks were found behind the right engine, and some streaks of oil were found behind the left engine. The right engine's crankshaft and rod bearing surface was found to be 0.010 inches under standard, but the rod bearings were standard size.
