On April 13, 1990, a Piper PA-11 (registration N4904M) was involved in an aviation accident near Knik Clacier, AK. Investigators recorded the probable cause as: The pilot's failure to maintain clearance from the terrain during a low approach, which resulted from elevator control jamming caused by foreign objects (channel locks) obstructing the flight controls. This summary draws on records from the U.S. National Transportation Safety Board (NTSB) historical archive.
An aircraft crashed after elevator controls jammed due to channel locks slipping under the floorboard during a low approach over a gravel bar.
What happened
The pilot elected to execute a low approach over a gravel bar prior to landing. While rolling out of a turn, the elevator control became jammed to the right. The airplane subsequently spiraled into the terrain and crashed.
The investigation
An examination of the wreckage revealed that a set of channel locks had slipped under the floorboard and become entangled in the flight controls.
Probable cause
The pilot's failure to maintain clearance from the terrain during a low approach, which resulted from elevator control jamming caused by foreign objects (channel locks) obstructing the flight controls.
