Event Sequence
According to the investigation, shortly after takeoff while the aircraft was between 300 and 400 feet above ground level, the engine lost power. Attempts to restore power were unsuccessful. During the subsequent emergency descent for a forced landing, the aircraft collided with trees and then struck the ground.
Fuel System Examination
The investigation determined that the fuel quantity was sufficient to sustain engine operation at the time of the incident. The fuel selector was found positioned to the 'BOTH' position. The left fuel tank, which was not damaged, was checked for contaminants; none were found. The right fuel tank was damaged, and no determination could be made as to whether it contained contaminants. The gascolator was drained, revealing approximately one teaspoon of water. Additionally, the fuel distributor cover was removed and found to contain rust inside.
Engine Examination and Testing
The engine was removed from the aircraft and sent to the manufacturer's facility for a proposed engine run. Several components of the engine were replaced due to impact damage. The engine was subsequently started and found to operate normally.
Maintenance and Certification
The last annual inspection of the aircraft was performed on December 7, 1987. The engine installed in the aircraft was not approved by the Federal Aviation Administration for installation in this particular aircraft type.
