Accident Overview

A ferry flight, authorized by a special airworthiness certificate, ended in an accident during takeoff from an intermediate stop. The pilot, who was also an Airframe and Powerplant (A&P) mechanic, had certified the aircraft as safe for the flight, a condition of the permit issued by the FAA. The flight originated in Colorado and was destined for California for maintenance.

Takeoff and Collision

A witness observed the aircraft use approximately 5,500 feet of runway before becoming airborne. After lifting off, the airplane failed to climb higher than 200 feet above ground level. It subsequently collided with rough terrain about two miles from the airport.

Upon examination at the crash site, investigators noted several non-standard repairs.
