On June 29, 1993, a Cessna 188B (registration N731DH) operated by Vaughn Flying Svc. Inc was involved in an aviation accident near Centre, AL. Investigators recorded the probable cause as: THE FAILURE OF MAINTENANCE PERSONNEL TO PROPERLY TORQUE THE CRANKCASE THROUGH BOLTS. This summary draws on records from the U.S. National Transportation Safety Board (NTSB) historical archive.
A pilot experienced a loss of engine power during aerial application and conducted a forced landing in a soybean field. During the landing roll, an intentional ground loop to avoid a residence led to main landing gear collapse and substantial aircraft damage. Engine examination revealed a broken crankshaft and main bearing shell with evidence of insufficient torque on crankcase through bolts.
Accident Narrative
A pilot engaged in aerial application maneuvers reported a sudden loss of engine power. The pilot executed a forced landing in a soybean field. During the landing roll, the pilot intentionally initiated a ground loop to avoid a residence located at the end of the field. This maneuver resulted in the collapse of the main landing gear and substantial damage to the airplane.
Engine Examination
Following the accident, an examination of the engine was conducted. The examination revealed a broken crankshaft and a broken main crankshaft bearing shell. Additionally, there was evidence of rubbing between the crankcase parting surfaces. The rubbing was attributed to insufficient torque being applied to the crankcase through bolts.
