Accident Details
The pilot reported departing Cedar City, Utah, without obtaining any fuel services. At takeoff, the fuel gauges indicated approximately 35 gallons total onboard. The planned route included a refueling stop at Page, Arizona, with a passenger drop-off at Marble Canyon.
While approaching Page, the pilot observed the fuel gauges indicating about one quarter in each tank. Instead of proceeding directly to Page for fuel, the pilot elected to first drop off the passenger at Marble Canyon. After landing at Marble Canyon, the passenger deplaned. The aircraft was then started and taxied to runway 21 for departure.
Engine Failure and Forced Landing
During the initial climb, at approximately 100 feet AGL, the engine surged and then quit. The pilot executed a forced landing off the runway. During the landing, the aircraft encountered rough terrain and obstacles, causing it to nose over.
Post-Accident Findings
According to the aircraft retrieval firm, examination of the aircraft after the incident revealed that the right fuel tank was "bone dry." The fuel selector was found positioned to the right tank. The left fuel tank contained an estimated 8 gallons of fuel. Fuel system continuity was confirmed, with no evidence of any line or tank disruption.
