Introduction
A multiengine flight training accident occurred when the aircraft, on its student's first multiengine flight, crashed into a field adjacent to the airport after a low pass. The flight had conducted three takeoffs and landings before departing the area and returning a few minutes later.
Sequence of Events
As the flight returned, the CFI transmitted their position over the radio. A ground-based witness, also a multiengine CFI, heard a gear warning horn sounding in the background of the transmission. The witness believed the flight was practicing a single-engine approach. The witness observed the aircraft descending to approximately 30 feet above ground level while overflying the approach end of the runway, then entering a shallow climb. Before reaching the departure end of the runway, the aircraft entered a left bank. The bank angle gradually increased until the aircraft became vertical, at which point it crashed into a field next to the airport.
Wreckage Examination
Examination of the wreckage revealed no preimpact mechanical malfunction. The left fuel tank selector was found in the 'OFF' position, while the right fuel tank selector was in the 'MAIN TANK' position. The left propeller appeared to have been in the feather position; the right propeller was twisted and bent.
Instructor Experience
The CFI had been issued a multiengine instructor pilot certificate 2.5 weeks before the accident. During that time, the CFI had provided approximately 8 hours of total multiengine instruction to another student.
