Incident Overview
An aircraft that was undergoing an annual inspection was returned to flight status for a charter flight without a maintenance release or logbook sign-off on the maintenance performed. Witnesses observed the airplane depart with both engines operating normally. Shortly after departure, the airplane turned and rolled to the right, then descended. It impacted the ground and burned.
Damage and Findings
The landing gear and wing flaps were found retracted. The cockpit was destroyed, and both engines sustained fire damage. The right propeller was found feathered; however, no identifiable problem was discovered with that engine. The aircraft was consumed by fire.
Operator and Pilot Status
The company operating the flight did not possess an air-taxi certificate. The pilot held only a private pilot certificate, which is insufficient for conducting charter operations.
Investigation Details
The investigation focused on the lack of proper maintenance documentation and the unauthorized charter operation. Despite the right propeller being feathered, the right engine showed no mechanical defects upon examination.
