Overview
A helicopter engaged in lifting a load of trees experienced an engine power loss while approximately 50 feet above the ground. The pilot attempted an autorotation to manage the descent.
Impact and Damage
Upon landing, the helicopter's right skid collapsed on impact with the terrain, causing the helicopter to roll onto its side. A witness observed black smoke coming from the helicopter during its descent.
Wreckage Examination
Examination of the wreckage at the site revealed that fuel was present at the fuel control unit, but no fuel was found in the line leading from the fuel control to the nozzle. The fuel tank contained approximately 150 pounds of jet fuel. Approximately ten drops of water were collected from the fuel sump screen.
Fuel Supply Investigation
An inspection of the nurse truck fuel supply at the site indicated that a filter installed at the pump was positioned at an incorrect angle. This misalignment allowed water to collect in the sump.
Engine Examination
The engine was examined at the factory and no mechanical anomalies were identified.
