A pilot experienced a sudden total loss of engine power at 300 feet AGL after takeoff. Post-accident examination could not determine the cause, but a later investigation found non-compliance with an airworthiness directive on the fuel pump drive coupling.

Accident Narrative

The pilot reported that an engine run-up performed before departure revealed no discrepancies. After takeoff, during the initial climb at approximately 300 feet above ground level, immediately after retracting the landing gear, the engine experienced a sudden and complete loss of power. The pilot maneuvered the airplane to the right to avoid hangars. While descending, the airplane first collided with trees and then struck the ground.

Post-accident Examination

Examination of the fuel delivery and vent systems showed no evidence of blockage. The fuel selector was found in the 'main' tank detent, with sufficient fuel present and no contaminants noted. The gascolator was drained and contained only 3-4 tablespoons of fuel, with no contaminants. The magneto switch and electrically operated auxiliary fuel pump were operationally checked and found to function properly. The engine was removed; after replacement of impact-damaged components, it was started and operated normally using the engine-driven fuel pump. No determination could be made as to the reason for the total loss of engine power.

Follow-up Investigation

Approximately 2.5 years after the accident, an investigation determined that the engine-driven fuel pump had been disassembled after the successful engine run. Airworthiness Directive (AD) 93-16-15, effective September 27, 1993, required replacement of the installed fuel pump drive coupling within 30 hours time in service from the effective date. This AD was not complied with by the inspector at the last annual inspection (May 1996); no entry was made indicating the AD was due in 30 hours. No annual inspections were performed and the airplane was not operated between the effective date of the AD and the May 1996 inspection. The AD was generated due to findings of mechanical damage to the ends of drive couplings in seven engine-driven fuel pumps; however, this condition was not evident in the accident fuel pump drive coupling. The airplane had accumulated approximately 38 hours time in service since the last annual inspection, of which 15.5 hours were by the accident pilot.
