Accident
During the takeoff roll of a tailwheel-equipped airplane, the bench-style front seat fell backward. The flight instructor and the student pilot were unable to reach the flight controls. The airplane subsequently ground looped to the left while at full throttle, and the right wing tip and right horizontal stabilizer struck the ground.
Investigation
An FAA inspector examined the airplane and reported that the seat adjustment pin had come out of its locking hole. This allowed the seat to slide back to a point where it became uncaptured and rolled over.
