Accident Overview
During the initial climb after takeoff, at approximately 200 feet above ground level, the pilot attempted to deactivate the particle separator system. Immediately afterward, the engine indicated low fuel pressure and subsequently lost power. The helicopter executed a forced landing, during which it touched down hard and rolled onto its side.
Pilot Report and Switch Configuration
The pilot reported believing he had accidentally operated the fuel shutoff switch instead of the particle separator switch. The two switches are positioned about 2 inches apart on the instrument panel. Both switches operate in the same direction along a common axis and differ only in shape. Post-accident investigation revealed the FUEL/OIL switch in the OFF position and the adjacent PART SEP switch in the ON position.
Mechanical Examination
Investigators found no evidence of pre-existing mechanical issues with the engine that could have contributed to the power loss.
Certification and Regulatory Context
The Kaman K-1200 received normal-category FAA type certification in August 1994. Federal Aviation Administration airworthiness standards for normal-category rotorcraft (14 CFR Part 27) require that cockpit controls be located to prevent confusion and inadvertent operation, and that means be provided to guard against inadvertent operation of each shutoff.
