During a soft-field takeoff attempt, the airplane lifted off at low airspeed, contacted both wings on the runway, then nosed down and departed the left side of the runway.

Accident Sequence

The flight instructor was providing dual instruction to a private pilot seated in the left seat. The instructor was guiding the student through a soft-field takeoff attempt with a nose-high attitude. Approximately 1,700 feet down the runway, the airplane lifted off at low airspeed. At about 2,000 feet down the runway, the left wing contacted the runway surface, followed by the right wing tip. Subsequently, the airplane nosed down and departed the runway on the left side, approximately 2,400-2,500 feet down the 3,400-foot runway.

Witness Observations

Witnesses reported that the airplane exhibited an over-rotated nose-high attitude during the takeoff run.

Flight Instructor Statement

The flight instructor stated that he assumed control of the airplane after lift-off and reduced power in an attempt to land. He noted that the elevator lost effectiveness when he reduced power, and at that time the airplane's wing dropped, resulting in a touchdown in a nose-low attitude.
