Incident Description
After preflight inspection, the second pilot (SP) started the engine and taxied the aircraft for takeoff. Following magneto checks, the SP initiated the takeoff roll. The aircraft accelerated normally, but at approximately 60 feet above the ground, the certified flight instructor (CFI) sensed a loss of engine power. The CFI immediately assumed control, extended the flaps, and began slipping the aircraft to manage the descent. During the slip, the CFI verified that the primer was in, the fuel pump was on, and the fuel selector was positioned to the right tank. The aircraft touched down beyond the departure end of the runway and subsequently went over a 30-foot drop-off, impacting nose-first.
Investigation Findings
Post-accident examination of the engine revealed no mechanical failures or anomalies. However, the fuel strainer was found locked in the open position. According to the pilot's operating handbook, the preflight procedure requires draining the fuel strainer. The open position would allow the engine fuel pump to ingest air, leading to a loss of power. No other issues were identified.
