Event Details
During the initial climb after takeoff, at an altitude of approximately 150 feet above ground level, the engine lost power. The pilot attempted to turn back to the departure airport. While maneuvering, the pilot lost control of the airplane, which entered a stall/spin and subsequently impacted a tree and the terrain.
Engine Induction System Examination
Post-accident examination of the engine induction system revealed that the scat-ducting supplying air to the heat-muff was obstructed. Two pieces of foam were found inserted into the inlets located on the lower engine cowl. The carburetor heat selection lever was in the full aft position, corresponding to maximum carburetor heat selection.
Passenger Account and Additional Findings
According to the passenger, the airplane had been washed just prior to the accident flight. No additional anomalies were found with the airframe or the engine that could be associated with any pre-impact condition.
Weather Conditions and Icing Potential
Based on a carburetor icing probability chart, the air temperature and dewpoint at the time had the potential for producing moderate icing at cruise power and serious icing at descent power.
Preflight Considerations
An FAA publication (FAA-P-8740-4A) states that the pilot is responsible for determining the aircraft's safe condition for flight, emphasizing the importance of a thorough preflight inspection. Another FAA publication (FAA-P-8740-44) notes that turning back is the worst possible action during a powerplant failure during climbout in a single-engine airplane.
