During the initial climb after takeoff, at an altitude of approximately 150 feet above ground level, the engine lost power. The pilot attempted to turn back to the departure airport. Engine Induction System Examination

Post-accident examination of the engine induction system revealed that the scat-ducting supplying air to the heat-muff was obstructed. Two pieces of foam were found inserted into the inlets located on the lower engine cowl.
