Accident Sequence

The pilot reported flying northbound along the Willamette River at 700 feet above ground level when the helicopter yawed left and the ENGINE OUT light illuminated. The pilot initiated an autorotation, instructed passengers to brace for a hard landing, broadcast a Mayday call, and selected a landing site on the river bank. The helicopter landed hard, seriously injuring the pilot; the passengers were not injured.

Investigation Findings

Post-accident examination of the engine revealed extensive application of a red sealer, consistent with high-temperature RTV, on several engine areas. Disassembly uncovered red sealant on interior surfaces and at various points in the engine oil system. The spur adapter gearshaft (SAG), which connects the gas producer turbine to the engine compressor, was found fractured, heat-distressed, and devoid of lubrication.

Engine Examination

An oil flow test of the oil delivery tube servicing the SAG showed no oil flow from the oil jet targeted for that component. Further inspection indicated that this oil jet was clogged.
