Incident Overview
A pilot aborted a mission due to snow, rain, and foggy conditions, leaving the rotorcraft overnight in an open field. The following morning, the pilot departed to return to base. Shortly after takeoff, the pilot radioed that power had been lost in the #1 engine. The pilot returned toward the departure site and, while at an altitude of approximately 200–300 feet above ground, power was lost in the #2 engine. The rotorcraft pitched nose down and landed hard.
Witness and Environmental Conditions
A witness at the scene reported a mix of rain and snow, freezing temperatures, and low ceilings at the time of the accident.
Post-Crash Examination
Post-crash examination of the rotorcraft revealed no mechanical malfunctions. Laboratory examination of fuel samples from several sources within the rotorcraft, including both airframe fuel filters, showed no significant anomalies. Both engines were test run at the manufacturer's facility with no indication of any mechanical problem. The #2 engine fuel control unit was flow checked and examined with no issues found.
System Findings
At the accident site, the RPM WARN circuit breaker (CB) was observed to be in the tripped position. Subsequent tests demonstrated that with this CB out, the engine failure and low rotor RPM warning lights and horns were inoperative. Additionally, the engine DEICE locking toggle switches, which control the engine anti-ice system, were observed in the off position at the accident site.
Flight Manual Information
The Flight Manual for the rotorcraft contained no Height-Velocity Diagram for a two-engine inoperative condition.
