What happened
A Robinson R-22 helicopter was involved in an accident while operating over a rice field. The pilot was attempting to maintain a hover when his attention was diverted from the aircraft's position relative to the terrain. As a result, the right skid of the helicopter struck a berm. The impact caused the rotorcraft to roll over onto its side.
The investigation
Following the accident, investigators conducted a post-impact examination of the helicopter to determine if any mechanical issues contributed to the event. The inspection focused on the airframe and systems for signs of failure or malfunction prior to the collision.
Findings
The examination of the helicopter failed to reveal any abnormalities or malfunctions that would have affected its operation. The accident was attributed to the pilot's loss of situational awareness during the hover phase, leading to an unintended ground contact.
