Engine Failure Investigation
The Australian Transportation Safety Board (ATSB) investigated an in-flight shutdown (IFSD) involving a Honeywell TPE331-11U-611G engine. The engine had accumulated 2,327.9 hours since overhaul and 2,473 cycles since overhaul.
Cause Determination
The ATSB determined that the cause of the IFSD was a below-platform stress rupture fracture of a 1st stage turbine blade, part number 3108125-1.
Request for Historical Data
As part of the investigation, the ATSB requested assistance from Honeywell to obtain information regarding the history of TPE331-11U 1st stage turbine blade failures and any preventative maintenance actions related to such failures.
Investigation Jurisdiction
The investigation is under the jurisdiction of the Government of Australia. The National Transportation Safety Board represented the United States under Annex 13 provisions. Further information may be obtained from the ATSB.
