Departure and Pilot's Report

The airplane took off from runway 19R shortly before the accident. Moments after becoming airborne, the pilot contacted the tower and requested to return to the airport, citing oil accumulating on the windscreen.

Witness Observation

A pilot who was taking off immediately behind the accident airplane reported seeing it turn right at approximately 300 feet above ground level. The witness described the airplane as appearing very slow, and shortly thereafter, it rotated toward the ground.

Postimpact Examination
