Incident Overview
While climbing through 9,000 feet, the pilot observed substantially reduced climb performance. Noticing a serious problem, the pilot saw the oil temperature red-lined, low oil pressure, and drastically reduced power. The autopilot was disconnected, and the nose was lowered to prevent a stall and cool the engine. The pilot contacted Minneapolis Center and reported the issue. When asked by the controller whether an emergency was being declared, the pilot affirmed and requested vectors to the nearest airport. Oil was seen on the left window. Descending through a scattered layer at 3-4,000 feet, the pilot became visual on the airport. With minimal airspeed and altitude, and power almost nil, the pilot aimed the aircraft toward midfield, intending to land without stalling or hitting trees. The aircraft was put down on uneven terrain at the south end of the runway, resulting in substantial damage.
Maintenance and Examination
A representative of the fixed base operator stated that three cylinders were removed and replaced. During the engine's operational check, no oil leaks were noted. The technician who performed the work also inspected the work. Examination of the wreckage found a disconnected oil line. This line went to the cylinder valve guides for cooling, connected at the outlet of the oil cooler from which oil is supplied to the valve guides. The representative explained that oil would begin to flow from the disconnected line once the engine warmed up and the vernatherm opened.
Human Factors Considerations
The representative stated that human factors were involved, as there was pressure to get the work done because of the owner's request. The representative noted that secondary inspectors will be used whenever possible and that the entire maintenance staff will be trained on human factors.
