Accident Overview
A helicopter en route to a pilot's residence located on a lake crashed during a dark night in visual meteorological conditions. The accident occurred as the helicopter crossed the lake, descending steadily toward the water. It hovered for approximately 1 to 2 seconds before falling vertically from an estimated height of 40 to 50 feet into the lake. The impact with the water took place about one-quarter mile from the intended landing area.
Sequence of Events
The helicopter was observed descending as it approached the lake. After a brief hover, it dropped vertically into the water. The pilot and a passenger survived the impact but subsequently died due to hypothermia resulting from prolonged exposure to cold lake water.
Investigation Findings
Investigators found no pre-impact mechanical malfunctions or anomalies that would have prevented normal operation of the helicopter. The flight profile before impact was consistent with a settling-with-power condition. The investigation noted that outside visual references for airspeed and rate of closure may not be available at night, particularly when approaching an unlighted landing area. A pilot's failure to monitor altitude, airspeed, and rate of descent during an approach can result in a settling-with-power condition.
