During takeoff from a 4,100-foot asphalt runway, the airplane experienced a stall warning after rotation, rolled left, struck the ground, and sustained substantial damage. Density altitude was 8,254 feet.

Accident Sequence

The pilot reported that the takeoff roll appeared normal, with acceleration to 80 knots and rotation to a normal attitude. The airplane was slow to lift off but became airborne with approximately half the runway remaining. Immediately after breaking ground, while the airplane was 3-5 feet in the air, the stall warning horn activated. The airplane rolled to the left, causing the left wing tip to strike the ground. Subsequently, the airplane rolled in the opposite direction, striking the runway with the right main landing gear. It then departed the left side of the runway, struck a dirt embankment, became airborne again, and impacted the terrain in a nose-down attitude. The airplane sustained substantial damage.
