Accident Sequence
The pilot reported that the takeoff roll appeared normal, with acceleration to 80 knots and rotation to a normal attitude. The airplane was slow to lift off but became airborne with approximately half the runway remaining. Immediately after breaking ground, while the airplane was 3-5 feet in the air, the stall warning horn activated. The airplane rolled to the left, causing the left wing tip to strike the ground. Subsequently, the airplane rolled in the opposite direction, striking the runway with the right main landing gear. It then departed the left side of the runway, struck a dirt embankment, became airborne again, and impacted the terrain in a nose-down attitude. The airplane sustained substantial damage.

The pilot reported performing a full engine run-up prior to the takeoff attempt and stated that all airplane systems were functioning properly.
Weather and Performance Data
Weather conditions at the time of the accident: clear skies, visibility 7 miles, wind from 200 degrees at 5 knots, temperature 98°F, dew point 34°F, and altimeter setting 30.14 inches of mercury. The density altitude was calculated at 8,254 feet. Using an aircraft performance chart for takeoff distance with flaps 0 degrees provided by the pilot, and entering raw weather data, the chart indicated a takeoff distance of approximately 1,950 feet. The chart assumed takeoff power set before brake release, landing gear retracted when a positive rate of climb was established, and a paved, level, dry runway surface.
