Accident Summary
A 35-year-old commercial-rated pilot was conducting practice maneuvers in a single-engine helicopter when the accident occurred. The pilot was performing a steep approach, a training exercise involving a high descent rate and low airspeed, during which the aircraft's tail rotor struck the ground after the flare was initiated too close to the surface.
Sequence of Events
According to the investigation, the pilot flared the helicopter at an altitude insufficient to prevent the tail rotor from contacting the ground. The impact caused the tail rotor drive shaft to fail, resulting in a loss of directional control. Subsequently, the rear portion of the right skid made contact with the ground, and the helicopter rolled onto its side.
Damage Assessment
Examination of the helicopter revealed substantial damage to the mast, tail boom, and the right skid, which was torn from the fuselage. The extent of the damage indicates a significant impact sequence consistent with a dynamic rollover event.
