What happened
On November 14, 2009, a Piper PA-28R-200, registration N4499T, crashed approximately 10 minutes after departing Woodbine Airport (OBI) in New Jersey. The flight was intended to proceed to Monroe County Airport in Bloomington, Indiana. The pilot, who was not instrument-rated, and a passenger were both fatally injured.
Prior to the accident, the pilot had delayed his departure for two days due to inclement weather. On the morning of the accident, instrument meteorological conditions (IMC) were present at the airport, with an overcast ceiling reported between 200 and 300 feet above ground level. Despite these conditions, the pilot completed fueling at 1030 and departed runway 31 at approximately 1040.
After takeoff, the aircraft climbed into the overcast. Witnesses on the ground reported hearing the airplane for several minutes, noting that the sound suggested the aircraft was continuously changing speed and direction. Radar and GPS data showed the aircraft performing multiple 360-degree and 180-degree turns at altitudes varying between 200 and 1,600 feet. Witnesses eventually heard a loud impact, and the wreckage was located about 3,500 feet southwest of the runway threshold.
The investigation
Investigators examined the wreckage and found the main debris field was approximately 1,260 feet long. The left wingtip and an outboard section of the left wing, along with part of the left stabilator, were located ahead of the main wreckage. Analysis of the wing and aileron showed damage consistent with airloads causing structural deflection, with no evidence of pre-impact mechanical failure or corrosion.
Engine examination revealed the engine was developing power at the time of impact. The landing gear was found in the retracted position, and the engine controls were in the full forward position. While the aircraft's vacuum pump and artificial horizon were examined, no evidence of pre-impact failure in these components was found. Maintenance records showed the aircraft had undergone an annual inspection in September 2009, though investigators found no evidence of recent required inspections for the altimeter and transponder.
