What happened
On April 10, 2010, at approximately 0656 UTC, a Tupolev Tu-154M, registration 101, crashed while performing an approach to the Military Aerodrom Smolensk "Severnyi" in Russia. The aircraft, operated by the Polish Air Force as flight PLF101, was destroyed upon impact and by a subsequent post-crash fire. The accident resulted in 89 fatalities among the passengers and 7 fatalities among the flight crew.
The investigation
Following the accident, the governments of the Russian Federation and the Republic of Poland agreed that the Interstate Aviation Committee (IAC) would lead the investigation. Although the aircraft was operated as a state aircraft, the investigation followed ICAO Annex 13 standards. Because the United States was the state of design and manufacture for the aircraft's Terrain Awareness and Warning System (TAWS) and Flight Management System (FMS) units, the NTSB provided support for the investigation activities.
