What happened
On June 14, 2013, at approximately 0810 UTC, a Bell 212 helicopter, bearing registration PR-HRZ, crashed in Tefe, Amazonas, Brazil. The aircraft, operated by an oil and gas exploration and production company, was performing a flight from a support base to Tefe Airport (SBTF).
Shortly after takeoff, the helicopter impacted terrain, resulting in the destruction of the aircraft. The accident resulted in 2 fatal injuries for the pilot and the passenger.
