Incident Notification
On October 20, 2013, an incident occurred involving a Boeing 757-200 aircraft. The Air Accident Investigation Unit (AAIU) of Ireland subsequently notified the National Transportation Safety Board (NTSB) of the United States about this incident.
International Investigation Cooperation
Under the framework of ICAO Annex 13, the NTSB appointed a U.S. Accredited Representative to participate in the investigation led by the AAIU. ICAO Annex 13 outlines the procedures for the investigation of aircraft accidents and incidents, and it grants the State of Manufacturer and Design the right to appoint an accredited representative to the investigating state's inquiry. In this case, the United States is the State of Manufacturer and Design for the Boeing 757-200.
Investigation Leadership and Information Release
The AAIU is the lead investigating authority for this occurrence. According to the notification, the AAIU will be responsible for releasing all investigative information. The NTSB's representative will assist the AAIU in the investigation.
