What happened

On October 21, 2014, at approximately 2005 UTC, a BAE Jetstream 41, registration HK4503, was performing a scheduled flight from Enrique Olaya Herrera Airport (EOH) in Medellin, Colombia, to El Caraño Airport (UIB) in Quibdó, Colombia. During the cruise phase of the flight, the aircraft experienced a stoppage of the No. 1 (left) engine.

The flight crew declared an emergency and initiated a return to Enrique Olaya Herrera Airport. The aircraft completed a single-engine landing at EOH without further incident. There were no injuries to the occupants.

The investigation

Following the landing, a visual inspection of the No. 1 engine was conducted. During this inspection, a hole was discovered in the engine gearbox. The investigation is being led by the Grupo de Investigación de Accidentes Aéreos (GRIAA) of Colombia, with the NTSB providing an accredited representative to assist as the state of design for the aircraft powerplants.
