DSB Notifies NTSB of 2015 Boeing 737 Incident

Incident Notification and Investigation Role On December 3, 2015, an incident involving a Boeing 737 airplane was reported to the Dutch Safety Board (DSB). The DSB subsequently notified the U.S. National Transportation Safety Board (NTSB) of the event. As the State of Manufacturer and Design for the aircraft, the NTSB was involved in the investigation process. The NTSB appointed a U.S. Accredited Representative to assist the DSB in its investigation. This appointment was made in accordance with the provisions of ICAO Annex 13. The DSB retained responsibility for releasing all investigative information related to the incident.
