Accident Overview
On June 4, 2016, at approximately 0815 local time, a Bell 206 helicopter with registration PK-UAF collided with terrain near Siriwo, Indonesia. The helicopter sustained substantial damage in the accident.
Occupants and Injuries
According to reports, there were four occupants onboard the helicopter. One occupant sustained fatal injuries, and three occupants sustained serious injuries.
Investigation
The National Transportation Safety Committee (NTSC) of Indonesia is conducting the investigation. In accordance with ICAO Annex 13, the National Transportation Safety Board (NTSB) of the United States has appointed a U.S. Accredited Representative as the State of Manufacture and Design.
