Incident Overview
On September 14, 2016, at Warsaw Chopin Airport (EPWA), a Bombardier DHC-8-400 operated by PLL LOT experienced a technical issue during engine start-up at the parking stand. At 15:00 UTC, the #3 HYD PUMP warning light activated. The flight crew followed the appropriate checklist procedures and subsequently shut down the engines.
Maintenance Actions
The crew informed the Maintenance Control Center (MCC) of the situation. Technical personnel conducted an inspection and replaced the #3 HYD PUMP relay, which was found to have a short circuit. After the replacement, the aircraft was returned to service. The scheduled flight departed with a delay.
Findings
An Aircraft Safety Report (ASR) was filed. The investigation determined that the cause of the incident was a short circuit of the contacts in the #3 HYD PUMP electrical relay. No injuries were reported among the 83 persons on board.
Preventive Measures
The operator recorded the event in their Safety Management System (SMS) database and classified it according to PLL LOT criteria. It will be included in safety performance indicators monitored per the Safety Management Manual.
