Incident Overview
On February 11, 2017, at approximately 1950 mountain standard time, a catering agent fell from Southwest Airlines flight 4152 at Phoenix Sky Harbor International Airport (PHX), Phoenix, Arizona. The aircraft, a Boeing 737-700 registered as N7822A, was preparing for a scheduled passenger flight to Sacramento International Airport (SMF) under 14 CFR Part 121.
Events
According to Southwest Airlines, passengers were boarding when the catering agent returned to the aircraft to deliver a box of peanuts. The agent parked the provisioning truck short of the airplane, leaving a gap between the truck and the aircraft. After entering the airplane and delivering the peanuts, the agent lost situational awareness and stepped backward, falling through the gap. The fall resulted in a fractured rib.

The flight crew consisted of a captain and first officer, along with three flight attendants. Several passengers were also on board. The flight was a regularly scheduled passenger service.
