Incident Overview
On 11 May 2017, at approximately 10:30 UTC, after completing a flight, the crew of an Embraer E195 began opening the left forward door (door 1L) at Warsaw Chopin Airport (EPWA). During this process, the emergency evacuation slide deployed from its housing but did not inflate. Passengers subsequently deplaned through the rear doors.
Investigation Findings
Investigation of the incident revealed that the door had not been properly disarmed prior to opening. The slide was removed and replaced with a serviceable unit in accordance with the Aircraft Maintenance Manual (AMM) task 25-65-01/400, Revision 30.
Cause and Contributing Factors
The probable cause of the incident was improper door disarming. Contributing factors included distraction and time pressure during the door-opening procedure.
Operator Actions
Following the incident, the aircraft was handed over to technical services, which replaced the emergency slide assembly. No safety recommendations were issued by the investigation body.
