On February 19, 2018, about 1125 Pacific standard time, a Piper PA-34-200T airplane, registration N14GQ, was destroyed when it struck the ground approximately 4 miles northwest of Minden-Tahoe Airport, Minden, Nevada. The commercial pilot received fatal injuries. The flight operated under Title 14 Code of Federal Regulations Part 91 as a personal cross-country trip from Meadows Field Airport, Bakersfield, California, departing around 0900. Visual meteorological conditions were present, and no flight plan was filed.

Flight Sequence

According to GPS data and ATC recordings, the pilot was in contact with Northern California Terminal Radar Approach Control. At 1119:41, he reported being at 14,500 ft msl about 10 miles southwest of Minden, inbound. At 1121:16, when about 7 miles west of the airport, the controller asked about the deviation north. After further exchanges, at 1123:07, the pilot stated he was orbiting to the south, maneuvering through a hole to see the airport. That was the last transmission.

GPS data showed the airplane then began a wide right turn. Altitude varied: at 1124:20, a spiral turn to 13,410 ft; nine seconds later, 12,493 ft; at 1124:49, a spiral of about 580° descending to 9,750 ft, the last reliable altitude. The airplane exceeded its maximum structural cruising speed (VNO) and never-exceed speed (VNE) after 1124:44. The flight load reached 3.95 G, exceeding the maximum 3.8 G limit. Data became unreliable after that, indicating the airplane broke up at about 1124:49 at around 9,500 ft msl (4,900 ft agl).

Witness

A witness about 2 miles north of the accident site heard a loud noise like a sonic boom, saw the airplane nose-diving with trailing smoke, but no flames.

Personnel

The pilot held an airline transport pilot certificate with multiengine land, commercial single-engine land/sea, private glider, and flight instructor ratings, plus an A&P certificate. His most recent second-class medical was September 24, 2017, reporting 6,000 total hours (50 in previous 6 months). Logbooks showed 5,945 total hours, with 4.9 hours in the accident airplane in the prior 6 months.

Aircraft

The 1978 Piper PA-34-200T was powered by two Continental engines: left TIO-360-EB-1B and right LTIO-360-EB-1B, each 200 hp, with Hartzell two-bladed propellers. The last annual inspection was August 1, 2017, at 3,948 total hours. It had a Robertson full-span flaps and spoiler system per STC SA154NW, removing ailerons. The airplane was equipped with a supplemental oxygen system, but no maintenance since April 2012 (hydrostatic check due March 2017, not performed).

At 1115, Minden Airport reported wind from 350° at 15 knots gusting 29, visibility 10+ sm, clear sky, temperature -2°C, dew point -13°C. Satellite imagery 30 minutes before the accident showed extensive cloud coverage with breaks. A pilot report 30 minutes after indicated clouds 9,500-13,000 ft msl about 10 miles east. Weather radar showed snow showers and light icing.

Wreckage and Impact

The airplane impacted inverted, wings-level, slightly nose-down. Main wreckage was concentrated; separated parts (nose cone, empennage, right wing outboard, left outboard wing) were found in a debris field about 0.5 mile south, 800 ft wide and 3,200 ft long on a 167° magnetic heading. Examination revealed overstress separations, no pre-existing corrosion or cracking. The oxygen system was found loose in the cabin; it could not be determined if the pilot was using it.

Medical and Toxicological

Autopsy determined cause of death as multiple blunt force injuries. The pilot had moderate coronary artery disease (50-75% stenosis in left anterior descending artery). Toxicology identified diphenhydramine (0.305 µg/mL in cavity blood), amlodipine, and atorvastatin. Diphenhydramine is a sedating antihistamine; its blood level exceeded the expected effect range (0.025-0.112 µg/mL) but note postmortem redistribution can increase levels up to three times. Amlodipine and atorvastatin are non-impairing.

Performance Study

The Appareo Stratus PRX V2 recorded GPS and attitude data. The flight was normal until the final minutes. Airspeed peaked at 220 kt at 1124:37, exceeding VNO (165 kt) and VNE (194 kt). The recorded load factor reached 3.95 G, exceeding the 3.8 G limit. The airplane broke up in flight.
