What happened
Following the event, the Civil Aviation Authority (CAA) of South Africa initiated an investigation. As the state responsible for the manufacturer and design of the aircraft, the NTSB appointed a U.S. Accredited Representative to assist the South African CAA investigation in accordance with ICAO Annex 13 provisions.
