Accident Overview and Flight History On February 3, 2019, at 1345 Pacific standard time, a Cessna 414 airplane, N414RS, was destroyed in an accident near Yorba Linda, California. The flight, conducted under 14 Code of Federal Regulations Part 91, resulted in fatal injuries to the pilot and four individuals on the ground, while two others sustained serious injuries. The aircraft departed Fullerton Municipal Airport (FUL) at approximately 1339. Radar and audio data indicated the pilot initiated a climbing left turn to the east. Although visual meteorological conditions (VMC) prevailed at departure, the weather transitioned to instrument meteorological conditions (IMC) with precipitation, a microburst, and rain showers over the accident area. The FUL tower controller had cautioned the pilot about deteriorating weather conditions approximately 4 miles east of the airport. ## Recorded Data and Witness Accounts About 5.5 minutes after takeoff, the aircraft reached an altitude of 7,800 feet above ground level (agl) before entering a rapid, descending right turn. Data from the Appareo Stratus 2S unit recorded an altitude of 5,500 ft agl at 1344:55, with a right bank angle of about 110° and a pitch angle of -35° nose down. Between 1345:03 and 1345:10, the pitch and bank angles fluctuated significantly as the aircraft descended to 1,000 ft agl. The final recorded data showed the aircraft at about 500 ft agl with a 40° nose-up attitude and a 30° right bank. Witnesses reported the aircraft exiting clouds at a high descent rate, followed by structural separation. One witness noted the aircraft remained in a 60° nose-down pitch for 4 to 5 seconds before a high-speed recovery attempt, during which the left horizontal stabilizer separated, followed by the remaining empennage and the left wing. A security camera captured the aircraft trailing black smoke and igniting. ## Meteorological and Environmental Conditions Meteorological data from the High Resolution Rapid Refresh model indicated a lifted condensation level at 627 ft agl and a convective condensation level at 1,608 ft agl. The environment was unstable to conditionally unstable from the surface through 6,500 ft agl. Satellite imagery showed broken-to-overcast cloud cover and bands of cumuliform clouds moving across the region. AIRMET advisories warned of moderate turbulence below 12,000 ft and mountain obscuration conditions. ## Wreckage and Impact Investigation The wreckage was scattered across a neighborhood in an area approximately 1,000 feet long and 800 feet wide. The left and right outboard wings separated near the outboard edge of the nacelles, with the right wing almost entirely consumed by fire. Examination of the wing spars revealed fractures with a dull, grainy appearance consistent with overstress separation. The one-piece horizontal stabilizer separated from the empennage, with the left half deformed downward about 80°. The vertical stabilizer and rudder also separated in multiple pieces. All primary flight control cables were broken at different locations, showing a splayed appearance consistent with overload. ## Mechanical and Medical Findings Examination of the engines and airframe revealed no evidence of mechanical malfunctions or failures that would have precluded normal operation. The Cessna 414 was equipped with deice boots and an S-Tec System 55x autopilot, though it was not certified for flight into known icing conditions. Toxicology tests on the pilot detected several substances, including sildenafil, sotalol, telmisartan, amlodipine, and acetaminophen, which are generally not considered impairing. Additionally, delta-9-tetrahydrocannabinol (THC) and its metabolites, as well as diphenhydramine, were identified in the pilot's system. The report notes that both THC and diphenhydramine can undergo postmortem redistribution, which may increase central blood levels compared with antemortem findings.
