On August 8, 2020, an incident occurred in Lobios, Ourense, involving a Canadair CL-215-1A10 with registration EC-HET. The accident resulted in 1 fatality and 1 serious injury.
Following the event, the Comision de Investigacion de Accidentes y Incidentes de Aviacion Civil (CIAIAC) of Spain notified the NTSB. Because the engines were manufactured in the United States, the NTSB appointed a U.S. Accredited Representative to assist the Spanish investigation under the provisions of ICAO Annex 13.
