Accident Overview and Flight History On August 10, 2022, at approximately 1459 Pacific daylight time, a Cirrus SR22, registration N379DH, was destroyed in an accident near Blyn, Washington. The pilot was fatally injured. The aircraft was operating as a personal flight under Title 14 Code of Federal Regulations (CFR) Part 91. The pilot's wife reported that he had departed Friday Harbor with the intention of flying to Olympia to rent a vehicle for a fishing trip in West Port, Washington. ADS-B data indicated the aircraft departed runway 16 at Friday Harbor Airport (FHR) around 1445, climbing to 1,100 feet mean sea level (msl). The aircraft maintained a southerly heading until 1458:14, when ADS-B contact was lost. Contact was reestablished at 1458:51 with the aircraft on an easterly heading, followed by a right turn to a southerly heading and a descent to 900 feet msl at 1459:07. A second loss of contact occurred, followed by reestablishment at 1459:13 at 1,100 feet msl. The final ADS-B target was recorded at 1459:19 at 1,400 feet msl, approximately 0.63 miles northeast of the accident site. NTSB Vehicle Recorders Laboratory data from the primary flight display (PFD) and multifunction display (MFD) showed the aircraft began a right turn from 169° magnetic to 238° magnetic over 11 seconds when ADS-B data stopped. The aircraft climbed to 1,460 feet msl and descended to 1,363 feet msl at the last recorded data point, located about 930 feet east of the accident site. Engine parameters showed power settings increased to 2,720 rpm and 29.2 inches of manifold pressure during takeoff. About 1 minute 42 seconds later, power was reduced to 2,440 rpm and 19.7 inches of manifold pressure, remaining consistent for about 7 minutes 36 seconds. Power was then reduced to 2,200 rpm and 16.4 inches of manifold pressure. About 54 seconds later, power increased to 2,280 rpm and 17.6 inches of manifold pressure, remaining consistent for about 3 minutes 18 seconds. Power increased over the last 36 seconds of recorded data to 2,680 rpm and 25.8 inches of manifold pressure. Cylinder No. 6's cylinder head temperature (CHT) reached 500° F 48 seconds after takeoff power was applied and remained at that temperature for 10 minutes 18 seconds before fluctuating between 487° F and 498° F. All other cylinders' CHTs remained consistent between 249° F and 283° F. Exhaust gas temperatures remained consistent among all six cylinders throughout the flight.
