Accident Details
On September 8, 2022, at approximately 1320 central daylight time, a Piper PA-28R-200 airplane, registration N29RM, was substantially damaged during an accident near Meridianville, Alabama. The aircraft was being operated as a Title 14 Code of Federal Regulations Part 91 instructional flight. The pilot under instruction and a flight instructor were on board and sustained minor injuries.
The pilot under instruction was at the controls when the airplane entered the traffic pattern at Huntsville Executive Airport (MDQ) to practice takeoffs and landings. Shortly after turning onto final approach for runway 36, the engine began to run rough, described as a cylinder “missing.” The flight instructor then took control of the airplane. He confirmed the fuel boost pump was on and the throttle was advanced. The engine subsequently stopped completely, though the propeller continued to windmill. Unable to reach the runway, the flight instructor executed a forced landing in a bean field short of the runway. After egressing, the pilots were met by first responders.
Examination Findings
An FAA inspector examined the wreckage at the owner's facility. Approximately 30 gallons of fuel were found onboard with no evidence of water or other contaminants in the tanks. The engine contained 8 quarts of clean oil, and the air induction system was unobstructed. Internal engine power and valvetrain continuity were confirmed. Suction and compression were observed on all cylinders; valve action was correct with no stuck valve. Engine control linkages were secure with no binding. The magnetos and ignition leads were secure, and the electric fuel boost pump functioned, pumping fuel. Fuel was observed in the line to the injector manifold. Top spark plugs showed normal color and wear. The fuel injector servo was removed and passed a diaphragm leakage check per the Lycoming troubleshooting guide. No internal damage was found, and all fuel injection nozzles were unobstructed. The inspector reported substantial damage to the fuselage and wings.
