What happened
On 2 June 2022, a Cessna 152, registration SP-AKW, departed from the EPRU aerodrome for an enroute flight. The pilot, a holder of a Light Aircraft Pilot Licence (LAPL(A)), was accompanied by a passenger. While the flight initially proceeded without incident, the aircraft's trajectory changed significantly approximately 30 minutes after takeoff. After passing through the area of Krzepice, the aircraft began a series of turns. Near the village of Rębiel and near Rębielice Królewskie, the aircraft entered a right-hand turn that transitioned into an uncontrollable spiral. The aircraft descended rapidly, accelerating along a tightening arc, and struck the ground at a high speed estimated at over 200 km/h. The impact destroyed the aircraft, and both the pilot and the passenger sustained fatal injuries at the scene.
The investigation
The PKBWL investigation examined several hypotheses to determine why the aircraft entered the spiral. Investigators analyzed the aircraft's mechanical condition, fuel levels, and the pilot's flight history. The investigation reviewed flight data from a FLARM device, which confirmed the aircraft's trajectory and the final spiral. The team also evaluated the possibility of engine failure, fuel exhaustion, pilot health issues, and potential interference from the passenger. The investigation also noted that while the pilot was legally permitted to fly solo, he did not meet the legal requirements to carry a passenger at the time of the accident.
Findings
- The aircraft's engine and airframe were in good condition, with no mechanical defects found that could have caused the accident.
- The fuel quantity was sufficient for the planned flight, and the engine was still operating at the moment of impact.
- The aircraft was within its proper loading and longitudinal balance limits.
- The pilot was not under the influence of alcohol, drugs, or stimulants.
- The pilot's flight experience was limited, having performed only about five hours of solo flight since obtaining his license.
- The investigation could not definitively confirm the cause of the spiral, though it noted the pilot's recent maneuvers included several turns that might suggest hesitation or a change in flight path intent.
