Incident Details

On July 2, 2024, at approximately 1330 Pacific daylight time, a Vans Aircraft Inc. RV-12IS airplane, registration N725FA, sustained substantial damage in an accident near Scappoose, Oregon. The pilot sustained serious injuries. The flight was conducted under Title 14 Code of Federal Regulations Part 91 as a personal flight.

Flight History

According to a review of Automated Dependent Surveillance-Broadcast (ADS-B) data, the airplane departed Grove Field Airport (1W1) in Camas, Washington, and flew to Scappoose Airport (SPB) in Scappoose, Oregon. Once at Scappoose, the airplane remained in the traffic pattern. The data indicated that the airplane completed four left-hand traffic patterns. The ADS-B data ended when the airplane was approximately half a mile short of the approach end of runway 15. At the last data point, the airplane's airspeed was about 60 knots and its altitude was about 50 feet.

Witness Observations

Witnesses located at the airport reported observing the airplane in a "hard left bank turn" with a nose-down attitude until it impacted terrain. The airplane came to rest on a taxiway east of runway 15.

Investigation Status

The airplane was retained for further examination. No probable cause has been stated at this time.
