Cessna 421C crash near Fairview, Oregon results in three fatalities

2 fatalities • Fairview, OR, United States

A private flight involving two pilots ended in a fatal accident after the aircraft struck powerlines and residential structures near Fairview.

What happened

On August 31, 2024, at approximately 10:30 Pacific daylight time, a Cessna 421C, registration N421GP, was destroyed during a flight near Fairview, Oregon. The accident resulted in 2 fatal injuries to the pilots and 1 fatal injury to a person on the ground.

Prior to departure from Troutdale Airport (TTD), the pilot requested a visual flight rules departure from runway 25, intending to remain within the airport traffic pattern. Radar data and air traffic control records indicate the aircraft departed runway 25 at 10:15. After the aircraft flew above the prescribed traffic pattern altitude, the controller queried the pilot's intentions. The pilot stated an intention to descend and remain in the pattern. Shortly after, the pilot reported experiencing "handling problems" and mentioned a "test flight" before a partial radio transmission ended.

Following the report of handling issues, the controller cleared the aircraft to land on runway 7. The second pilot on board responded that they would try to land but were unsure if they could. No further communications were received. Radar data showed the aircraft performing a left descending turn approximately 2 miles south of runway 25 until radar contact was lost.

Surveillance video captured the aircraft in a descending left turn with its landing gear extended. While on a southeasterly heading, the left wing struck high-tension powerline wires. The aircraft continued southeast, impacting a mature oak tree and several residential structures, which subsequently led to a post-impact fire.

The investigation

Examination of the accident site identified the first point of contact as high-tension powerline wires located approximately 110 feet in height. The debris path, containing aircraft fragments, vegetation, and residential debris, measured approximately 703 feet long and 477 feet wide, oriented on a magnetic heading of 120 degrees. All major structural components of the aircraft were found within this debris path, with the main wreckage located inside a residential structure. The wreckage was moved to a secure location for further examination.
