What happened
On the morning of 18 April 1974, a Piper PA-23 Series 250 Aztec, registered as G-AYDE, was operating a private flight from Luton Airport. Simultaneously, a BAC One-Eleven Type 518, bearing registration G-AXMJ, was preparing for departure on the same airfield. As the commercial jet commenced its takeoff roll, the smaller piston-engine aircraft was still airborne in the vicinity of the runway environment. The two aircraft collided mid-air over the airport grounds. The impact resulted in the destruction of both aircraft and the loss of all lives on board.
The investigation
The Air Accidents Investigation Branch conducted a thorough examination of the incident, focusing on the operational procedures at Luton Airport at that time. Investigators analyzed air traffic control communications, radar data, and the flight paths of both aircraft. The inquiry sought to determine how the Piper Aztec remained in the airspace while the BAC One-Eleven was accelerating for takeoff. Key areas of scrutiny included the effectiveness of visual scanning by the pilots, the clarity of radio instructions issued by controllers, and the overall traffic management protocols in place during that period.
Findings
The investigation established that the primary cause of the accident was a failure to maintain separation between the two aircraft. G-AYDE and G-AXMJ were operating in close proximity without adequate awareness of each other's positions. The BAC One-Eleven, a large commercial jet, was cleared for takeoff while the Piper Aztec, a smaller private aircraft, was still maneuvering in the airfield area. The findings indicated that neither the pilots nor the air traffic controllers had sufficient situational awareness to prevent the collision. All 14 people on board both aircraft were killed. The lack of effective communication and the limitations of visual separation procedures at Luton Airport contributed significantly to the tragedy.
Safety action
Following this accident, significant changes were implemented regarding air traffic control procedures at busy airports. Recommendations were made to enhance radar coverage and improve communication protocols between controllers and pilots operating in mixed traffic environments.
