Occurrence Details
On 19 August 2023, at approximately 10:30 local time (1430 UTC), a Piper PA-31-350 aircraft, United States registration N122AW, operated by Aztec Airways as flight AZY 422, was involved in a serious incident at North Eleuthera International Airport (MYEH), Eleuthera, Bahamas. The flight was conducted under Title 14 Code of Federal Regulations Part 135 commercial rules. The other aircraft involved was a Cessna Citation 680A, registration N108BM.
According to reports, N108BM was established on final approach for runway 07 at MYEH, less than 5 miles from the threshold, when N122AW taxied onto the active runway to position for departure to Fort Lauderdale International Airport (KFLL), Florida, USA.
Reaction and Outcome
As a result, the pilot in command of N108BM aborted the approach and executed a go-around maneuver. The pilot of N122AW subsequently exited the runway and taxied back to the ramp area. N108BM then reestablished the approach and landed without further incident.
Conditions and Contributing Factors
The weather at the time was Visual Meteorological Conditions (VMC) and was not considered a factor. No injuries were reported, and neither aircraft sustained damage.
Investigation Findings
Based on reports received, it appears that miscommunication may have contributed to the occurrence. The Aircraft Accident Investigation Authority (AAIA) of the Bahamas classified the event as a serious incident (MAC – near miss) and has completed its investigation. The investigation focused on factual circumstances and did not assign blame or liability.
