What happened
During the early morning hours of 10 May 2019, a collision occurred on the apron at Toronto/Lester B. Pearson International Airport in Ontario. The incident involved a de Havilland DHC-8-311, registration C-FJXZ, operated by Jazz Aviation LP as flight JZA8615, and a Rampstar fuel tanker operated by Menzies Aviation.
At approximately 01:33 Eastern Daylight Time, the aircraft was operating in darkness with 52 passengers (including three infants), three crew members, and one flight deck observer on board. Following the impact between the aircraft and the fuel truck, an evacuation of the aircraft was conducted. First responders, including airport officials and aircraft rescue and fire-fighting personnel, assisted the passengers and crew to the terminal building. While the collision caused 15 minor injuries, including one infant and one crew member, there was no fire or fuel spillage resulting from the event. The aircraft's emergency locator transmitter failed to activate during the incident.
The investigation
The TSB investigation focused on the circumstances surrounding the collision between the aircraft and the fuel tanker on the airport apron. The investigation examined the operational environment during the hours of darkness and the movements of both the Jazz Aviation flight and the Menzies Aviation fuel tanker.
