What happened
On 12 October 2018, a Boeing 737-4Q8, registration G-JMCR, was operating en-route to East Midlands Airport. The flight was subject to an investigation by the AAIB following an occurrence recorded under reference EW/C2018/10/03.
The investigation
During the review of the previously published AAIB Bulletin No 10/2018, a discrepancy was identified within the synopsis of the report. The original text incorrectly stated the number of safety recommendations resulting from the investigation.
Findings
An audit of the report's conclusion revealed that the initial publication erroneously claimed six safety recommendations had been issued to the operator regarding their safety management system. The corrected record confirms that five safety recommendations were actually directed to the operator's safety management system, alongside one recommendation directed to the Civil Aviation Authority.
