Fatal Engine Failure and Loss of Control in Robin DR400-180

Casualties unknown • French is the work of reference., FR

A flight involving four occupants ended in a fatal sea impact off the coast of Propriano after an engine failure and subsequent loss of control.

What happened

On the evening of October 12, 2023, a Robin DR400-180, registration F-GIKS, departed Propriano aerodrome for a return flight to Cannes-Mandelieu. The aircraft, operated by Aéroclub d’Antibes, was carrying a pilot and three passengers. Shortly after takeoff from runway 27, the aircraft performed a sharp right turn over the nearby beach.

During the climb, security camera footage captured a release of dark smoke from behind the aircraft. Witnesses reported hearing fluctuations in engine speed, and the aircraft appeared unstable on its roll axis. As the plane progressed toward Propriano bay, it entered an S-shaped flight pattern, characterized by alternating turns as if the pilot were attempting to level the wings. Following a second right turn, the aircraft pitched nose-down and entered a spin at an altitude of less than 200 meters. The aircraft struck the surface of the sea, resulting in four fatalities and the destruction of the aircraft.

The investigation

Investigators analyzed security camera footage from both Cannes-Mandelieu and Propriano aerodromes, as well as witness statements and wreckage. The examination of the wreckage, which was recovered 8 meters underwater, showed that the flight controls were intact and the trim was in a neutral position. The propeller showed no evidence of engine torque at the moment of impact.

While the investigation could not definitively identify a specific mechanical cause for the power loss, the presence of smoke and the difficulty the pilot experienced during engine start-up suggested a potential carburettor problem. The investigation also noted that the pilot did not stop at the holding point to perform standard pre-takeoff checks, though it was possible these were performed while taxiing.

Findings

  • The aircraft experienced a loss of engine power following a visible smoke release.
  • The pilot likely attempted an adapted circuit to return to the aerodrome.
  • The aircraft entered an asymmetric stall during a maneuver, leading to a loss of control.
  • The low altitude at the time of the stall prevented the pilot from avoiding the impact with the water.

Probable cause

The aircraft experienced an engine power reduction, possibly linked to a carburetor issue, which led to an asymmetric stall and subsequent loss of control during a maneuver at low altitude.
